When I've had a flush base by the spindles I've always gave my customer the option of either a trim or have it neatly folding under itself and pinned.
Could have been made a whole lot neater on that top step, looks like it's been rushed, fired a load of pins in there and done one. The trim...

I dry fit 2 blocks round the island/wall with drawn lines. Bit of maneuvering here and there but once you done a few its easier.
