Defo the ply header then. Maybe a different batch/board. If it’s just one it’s a easy fix. Probably need new planks tho
It seems wider than a ply line. The subfloor below all the same? Maybe just one bad board if they are laid across the floor that way
Is it in line with the extension or the whole floor complete?
That ply is dried to 8-10% most floor boards I take a reading of are well over that.
Is it level? Maybe it’s got glue sitting in it
Swelling ply is usually moisture. The subfloor could have a higher moisture content than the ply. Maybe too much water in the feather finish. Or...
If so you’d be better off speaking to Wakol for a system
Is it solid oak?
How old is your house? Was the subfloor moisture tested? Was the plywood acclimatised?

We always use flexi fix for nosings. Do try and get at least 2 screws in but you need a wide nosing or at least drilled along the back. So...

If your putting down a sand and cement why wouldn’t You just put a visqueen dpm sheet under it? You could then dpm the new screed after it’s cured...
