Looks like a sheet product called marley vinyl flooring is the best product. Could still be installed on a jumpax or loose laid.

I would get a damp survey done. Could be a bad drain and or guttering. Best to make sure theres not more of a problem than just rising damp

Yeah, get some better gripper rods. We prefer diamond gripper. Try creasing the backing, folding it to break it. Also crease it on to the gripper....
Did they use two coats of primer? Do you have under floor heating? How was the moisture reading taken. How long did you leave the new screed to dry?
